Included with this letter are the forms that you will need to fill out prior to your first visit to our clinic.  Please print the forms on plain paper. Fill out all questions that you can answer using black ink only. 
Please wear (or bring with you) clean socks to all appointments.  
When you come into the office please help yourself to a pair of slippers to wear during your visit. Clean socks are not provided so please bring your own to wear in the slippers. The slippers are important for some of the exams that Dr. Hedrich performs and are not to leave the main office. Please do not wear the office slippers to the restroom.
Please wear a plain, crew neck t-shirt without metal or bead embellishments should x-rays be needed. Please remove all jewellery from your head and neck.  Our office is scent free so please do not wear perfumes or scented products. 
We do have a dress code.  Clothing must be clean and modest. Ladies, please do not wear revealing tops, dresses or skirts. Dresses and skirts mid-calf and longer are OK. Please wear appropriate clothing for Shockwave or acupuncture which allows access to the back, legs and arms such as shorts and tank tops. You may need to remove some clothing items and blankets are available for coverup. In the chiropractic office, acupuncture is not offered as a stand-alone service. It will be offered in conjunction with appropriate chiropractic care.
We require that you would turn off your cell phone while in the office. If you need to use it we ask that you step outside our office to do so.
Payment is due at the time of service. We accept cash, cheque, bank card, Visa or MasterCard.

The address of the clinic is: Suite 204, 740 - 4 Avenue South, Lethbridge. It is on the 2nd floor of the Professional Building east of the main post office and south of the Alberta Treasury Bank (the old Paramount Theatre.) There is 2 hour metre parking on 8th street and a side door to enter the Professional Building with stairs that take you right to the second floor or through the front door you will find stairs and an elevator in the lobby on 4th Avenue.
